As an outpatient RN for our Brain Injury Day Treatment facility you will support patients clinically, and in a community based reintegration program.
Typical schedule is 8 to 4:30 Monday through Friday with no weekends or holidays.
Main work location will be Sacred Heart Outpatient, First floor of Women's Health 2323 North Lake Dr in Milwaukee.

What You Will Do
What You Will Do
As an associate with Columbia St. Mary's-WI, you will have the opportunity to educate, advocate, and coordinate care for patients in outpatient medical and comprehensive therapy programs for patients with brain injuries, stroke and other complex medical conditions.
· Implements and monitors patient care plans for patients enrolled in the Brain Injury Day Treatment Program.
· Serves as a primary coordinator of all disciplines for well-coordinated patient care.
· Notes and carries out physician and nursing orders.
· Assists with in-office procedures performed by physiatrist, including Botox injections and EMG/nerve conduction studies.
· Assesses and coordinates patient's home needs with members of the healthcare team.
