What You Will Do

As a Senior Director, Care Management with Ascension Care Management, you will have the opportunity to lead care management operations and strategy across assigned areas to ensure meeting performance metrics and other quality initiatives.

Responsibilities:

  • Leads the organization-wide cross-continuum care management and optimized site of service initiatives.
  • Leads care management operations to provide high quality, cost effective services.
  • Creates and develops new workflows and processes to improve care management.
  • Measures effectiveness and takes steps to ensure continuous performance improvement.
  • Develops and aligns strategies and initiatives from data analytics, trends and other findings that drive performance and goal achievement.
  • Responsible for developing enterprise wide care management strategy.
  • Assesses and optimizes revenue opportunities that drive performance in clinical operations, including creating new programs that support these objectives.
  • Engages in development of staff and builds talent to ensure continuity of clinical operations.

What You Will Need

Education:

  • Bachelor's degree required.
  • Master's degree preferred.

Work Experience:

  • 7 years of experience required.
  • 10 years of experience preferred.
  • 5 years of leadership or management experience preferred.
  • Experience with ambulatory care and population health is strongly preferred.
  • Multi-system experience is strongly preferred.
