What You Will Do
In this role you will be part of our outpatient / population health team! We have a unique model for our population health program here at St. Agnes where teams of three (RN Case Manager, Social Worker and Care Coordinator) work together in true partnership to serve our patients. As our Diabetes Educator you would support and partner with these teams specifically with patients that are diabetic or at risk of becoming diabetic. You would provide short term intervention to stabilize the patient, establish goals and get them connected to the level of intervention appropriate for the individual. The long term case management is handled by the teams but you will be instrumental to getting these patients off on (or back on) the right foot!
We feel our team is able to make a bigger impact in helping our patients through a more personal approach. Unlike telephonic population health programs, here at St. Agnes you will meet with patients in person as you float between our primary care practices (3 in Baltimore, 1 in Catonsville, 4 in Howard County). Your home base / office will be at one of these practices or on the campus of St. Agnes based on what makes the most sense for the selected candidate as a launching point.
This program just started in 2018 so you will enjoy the excitement and change of a start up within the stable environment of an employer that has been in Baltimore for generations. This team is big enough to have a lot of energy but we are small enough that we know our linkages well. Our team collaboration is unmatched! This group loves each other and their work.
This is a full time, salaried opportunity. Work hours will primarily be Monday - Friday 8AM - 4:30PM but some flexibility is allowed / needed to accommodate patient needs. Evening and weekend hours are very rare. What You Will Need
Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date.
Diploma from an accredited school/college of nursing required.
At least two years of experience working as an RN in a role where you were significantly exposed to and providing patient education surrounding diabetes is strongly preferred.
Experience and Certification as a Diabetes Educator (CDE) preferred.
