What You Will Do
In this role you will be part of our outpatient / population health team! We have a unique model for our population health program here at St. Agnes where teams of three (RN Case Manager, Social Worker and Care Coordinator) work together in true partnership to serve our patients.
We feel our team is able to make a bigger impact in helping our patients through a more personal approach. Unlike telephonic case management programs, here at St. Agnes our team is based on-campus at our Health Institute. They meet the patients live at their doctor’s appointments or in their homes so truly develop a rapport.
In this role you would be a people manager and project manager helping to guide this team, implement structure and identify / create best practices. This program just started in 2018 so you will enjoy the excitement and change of a start up within the stable environment of an employer that has been in Baltimore for generations. This team is big enough to have a lot of energy but we are small enough that we know our linkages well. Our team collaboration is unmatched! This group loves each other and their work.
This is a full time, salaried opportunity. Work hours will primarily be Monday - Friday 8AM - 4:30PM but some flexibility is allowed / needed to accommodate the needs of the team / program. Evening and weekend hours are very rare. What You Will Need
Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date.
Diploma from an accredited school/college of nursing required.
Prior experience as a Case Manager in an outpatient setting with a broad understanding of the different resources available to patients strongly preferred.
Prior leadership / management experience preferred.
Project management experience preferred.
Reporting and data analytics experience / skills a plus!
