Additional Job Details: .9 FTE / Days. Hours: 7AM-7PM; 3 12 hours shifts. Every other weekend. 1/2 of holidays
The 5th Floor is a 36 bed Intermediate Care unit. The staff provides excellent care and service that includes comprehensive assessments, pain management/symptom control, medication administration, discharge planning, patient and family education, psychosocial/spiritual support.

What you will do:
The staff starts each shift with a change of Shift Huddle, then Bedside Report followed by Hourly Rounds. These best practices help to ensure high quality and patient satisfaction. The Nursing staff work with multidisciplinary team which includes: Hospitalists, Case Managers, Social Workers, Pharmacists, Cardiac Rehab Nurses, OT, PT, Speech Therapy, Chaplains and the Palliative Care team. This team works together to meet patient’s needs through Multidisciplinary Rounds each week day.
