What You Will Do

In this positoin you will be part of a team of RNs and Social Workers who coordinate the overall interdisciplinary plan of care for patients from admission to discharge. 

Responsibilities:

  • Creates plan for care across the continuum, integrating patient/family preferences and values.
  • Monitors patient care through assessments, evaluations and/or patient records.
  • Advocates for resources and removal of barriers.
  • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
  • Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

This is a per diem position.  You will support the team by covering while staff are on vacation.  We expect five to ten 8-hour shifts per month.  Primarily Monday - Friday day shift and up to four weekends per year.  

What You Will Need

  • Required:  Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date

  • Required:  Diploma from an accredited school/college of nursing

  • Strongly Preferred:  A minimum of three years of work experience as a Registered Nurse in an acute care hospital setting

  • Preferred:  Previous experience in discharge planning, or case management in an acute care hospital setting 

  • Preferred:  BLS Provider credentialed from the American Heart Association (AHA). 

  • Preferred:  Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
