Case Management Coordinator, Ascension St. Vincent's Clay
Full-time Days, 40 hours weekly
Summary: Provides health coaching to qualified participants geared toward reducing or eliminating high-risk behaviors. Coordinates access to health-related services by assisting clients with utilizing available resources.
Contacts participants eligible for enrollment in the program and provides necessary health coaching to reduce or eliminate high-risk behaviors.
Establishes goals with clients to improve health and quality of life in both group and individual settings.
Supports external coaching programs by assisting with program needs assessment, recommending and implementing coaching program designs and evaluating and revising coaching programs as needed.
Facilitates various coaching groups and teaches classes on health coaching themes.
Identifies potential clients, completes initial needs assessment, and maintains follow-up to ensure appropriate guidance to health services.
Provides informal counseling, education, and social support to clients. Assists clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.
Develops and maintains strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members).
Maintains accurate client records by documenting all contacts, services provided, and outcomes to track all clients and services for future reference.
Assists members with member-specific needs, in-person, virtually and by phone, including health coaching, assistance with application for public aid, payer specific programs, medication access, and/or disability benefits.
Transports donated items, food boxes, and other resources to members.
Performs home visits for non-medication-related concerns (e.g. safety checks or social visits).
Performs non-clinical member assessments as needed.
