A New Unit
You will work in our newly designed innovation unit. In this setting, the entire care team is centrally located to foster communication and cohesion. Our new associate break room provides opportunity to recharge by providing a home-like feel with state of the art technology to allow you to connect virtually with your family when you can’t physically be with them. We believe that in order to truly care for your patients, you first need to take care of yourself. This new unit and team makes every effort to uphold that tenant.
What You Will Do
As a member of the interdisciplinary care team you will be responsible for coordinating holistic pharmacy services across the continuum of the medication use process. Your duties will be divided between the clinical monitoring of patients, participating in interdisciplinary rounds, medication therapy management, medication safety improvement, drug policy development/revision, medication discharge planning, and oversight of research and investigational drug protocols as well as drug distribution services for their assigned unit.
As a Medication Management Pharmacist, you will also be involved as a preceptor for pharmacy residents and students.
Develop, manage and mentor pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as determined appropriate based on patient population and literature guidelines
Sustain the hospital formulary and minimizing non-formulary procurements by utilizing therapeutic interchanges and promoting rational drug therapy selection
Identify, design, and analyze drug utilization reviews to promote rational drug therapy and determines action plan based on results.
Develop and manage specialty pharmacy practice programs as related to clinical pharmacy services.
Accurately compound and dispense pharmaceuticals as needed
Provide clinical consultation and clarification to practitioners.
Support participation in daily multidisciplinary and bedside rounds.
Develop and actively participate in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses and physicians.
Responsible for routine publication of the pharmacy newsletter and/or other forms of written and electronic communication
Responsible for the development, implementation, and monitoring of pharmacy operations initiatives (operational/clinical) to maximize clinical productivity.
