What You Will Do

As an Clinical Coordinator (Admissions) with Ascension Living's Our Lady of Peace in Lewiston, NY, you will have the opportunity to coordinate marketing and admissions activities and helps ensure a smooth transition process for patients transferring into community.

Responsibilities:

  • Markets and promotes community in order to maintain acceptable census and community profitability.
  • Partners with referral sources to identify prospective residents.
  • Makes decisions regarding pre-admission assessments to ensure the clinical appropriateness for community and to determine if patient meets Medicare guidelines.
  • Collaborates with hospital staff to establish a coordinated plan of care, coordinate admission process and ensure appropriate resources are obtained for a successful transfer.
  • Collects and provides monthly data reports to leadership regarding inquiries/referrals, pre-admission evaluations/outcomes, and enrollment.

What You Will Need

Licenses/Certifications/Registration:

  • Required Credential(s):
    • NONE

Education:

  • REQUIRED: High School Diploma/GED with 2 years of experience OR Associate's Degree OR Bachelor's Degree

Work Experience:

  • 3 years of experience (PREFERRED)
