Live at Home includes both long-term care coverage and care coordination. Coordination of care can be as challenging as paying for it, especially when it falls on a family member. Knowing that you have a personal care manager in your corner to help you navigate future healthcare is a comfort.
If members have a physical or cognitive change after joining the program, Live at Home steps in with homecare services that include home chores, transportation and assistance with activities of daily living. Homecare is fully covered by the program.
If a member can no longer stay in their home safely, they have priority access to Alexian Village Assisted Living and Health Care Center, and Live at Home covers 75% of the cost.
Live At Home is a pre-plan for future health needs, and enrollment qualifications include being healthy, aged 55 through 85 and without dementia, mobility issues or debilitating degenerative disease.
