What You Will Do
The Medical Director, Post-Acute Care (MD) will play a key strategic role as part of both the Ascension Living Executive Community (ALEC) and Ascension Clinical and Network Services (CNS) teams as they implement high quality continuity of care across the Ascension continuum. Additionally, the MD will provide input and be accountable for key clinical, operating, and financial indicators for Ascension Living, the National senior living subsidiary of Ascension. The MD will provide crucial physician leadership for the organization and will serve as a clinical dyad partner to the Chief Nursing Officer (CNO) in areas of quality of care, cost of care, resident and provider experiences. The MD will work in conjunction with the CNS Chief Medical Officer and other operational roles in support to deliver an integrated care experience.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Provide Medical Director oversight, expertise and leadership to ensure the delivery of quality healthcare services to Ascension Living’s communities
Partner with Ascension post-acute care partners and narrow network providers to continuously improve transitions of care, with the quadruple aim as a key performance indicator
Participate with Ascension leadership planning to implement post-acute business and clinical strategy.
Work in close partnership with other clinical leaders within Ascension to build highly effective delivery models, promoting the value of service coordination to improve lives and lower the cost of health care for the elderly.
Assists in advancing physician involvement in clinical systems within Ascension Living communities to a level of quality exceeding government requirements.
Keep abreast of emerging models in health care delivery; identify and define new and innovative strategies to achieve business goals and objectives.

What You Will Need
EDUCATION AND/OR EXPERIENCE
Medical degree, M.D. or D.O., or equivalent.
Minimum of 5 years of clinical and administrative experience in a relevant, multi-facility, multi-site, complex care delivery environment preferred.
A minimum of 5 years professional post-residency experience in direct patient care.
Experience leading multi-disciplinary teams of clinicians and functional specialists in a successful change program involving significant clinical process, practice change, and performance improvement.
CERTIFICATES, LICENSES, REGISTRATIONS
Certified by the American Board of Medical Specialties in Internal Medicine or Family Medicine AND Geriatric Medicine preferred.
Experience in palliative and hospice care preferred

Must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and ministry leadership.
The incumbent must demonstrate the ability to communicate clearly and concisely on oral and written form.

Working knowledge and proficiency with Microsoft Office is required.
