As as our Clinical Pharmacy Specialist – Infectious Disease, you will lead our Antimicrobial Stewardship Program. You will work to optimize clinical outcomes, promote cost effective therapy educate practitioners and improve care while minimizing unintended effects of antimicrobials in the hospital. Additionally, you will work to integrate our Antimicrobial Stewardship Program with patient safety and quality improvement initiatives.
Integrate pharmacy, infection control, information system, and infectious disease medical staff in carrying out antimicrobial stewardship program.
Involved in the development, implementation, evaluation and continuous evolution of clinical pathways in infectious disease management programs.
Evaluating and maintaining quality initiatives by accrediting organizations pertaining to antimicrobial program.
Actively participates in committees, task forces, projects, and provides leadership as assigned. Including Co-Chair of the Antibiotic Stewardship Committee.
Maintenance and dissemination of drug information pertaining to infectious disease, but not limited to, in-service education of hospital personnel and pharmacy staff
Exercises professional judgment in imparting therapeutic and drug information to health professionals.
Inspects pharmacy records to monitor drug therapy for appropriateness and safety consistent with a high level of patient care.
Assists in developing and maintaining reports and information for pharmacy management, hospital administration, and antimicrobial stewardship team.
Participate on a National level with the Ascension Antibiotic Stewardship Program.
Serves as a consultant to hospital Pharmacy and Therapeutics Committees and other hospital sponsored clinical and performance improvement committees.
Completed a PGY1 ASHP Accredited Residency Program
BLS and ACLS certifications or capable of obtaining certifications are required.
Three years of Pharmacist experience in a hospital setting
